Scrip Designation Form

Your designation may be changed one time per year, and it must be received by 1/31/2023, no exceptions. If no designation is selected or turned in, 100% of your earnings will go to the Restoration Fund. 

In order to keep contributing to the Tuition Fund, you must complete a new form at the beginning of each year.
